Your participation in this short 15 minute (or less) survey helps us at Superior Street to understand what is working well and what areas need improvement.
In addition, classrooms with a 100% completion rate will be entered into a drawing to receive popcorn and a movie on a Friday after Spring Break. One classroom from UTK-2 and one classroom from grades 3-5 will receive this special treat. Each family only needs to complete the survey once to cover all of their children at Superior Street. Once you complete the survey, please either send in a physical copy of the “Thank You” page OR take a screenshot of the “Thank You” page and send it electronically to your child’s teacher to have your completion recorded.
