Your information will help our school team better understand your child’s strengths and needs and determine whether additional Tier 2 supports may be helpful. Tier 2 supports are targeted, positive interventions designed to provide students with additional instruction, structure, encouragement, feedback, and opportunities to develop skills for success.
Submitting this form does not automatically place a student into a Tier 2 intervention. Our team will review the information provided, consider additional school-based information, and determine the most appropriate next steps. We will follow up with you as needed.
